A short, specific description can help the responsible organisation investigate. Your experience is enough to start a conversation; you do not need to diagnose the technical cause.
Find the right recipient
Look for Accessibility, Contact, Feedback or Support on the organisation’s website or app.
If you cannot find an accessibility contact, use a general support channel and ask for your message to be forwarded to the team responsible for the service.
If the task is urgent or you cannot use the reporting channel, request an accessible alternative for completing it. Ask for a reply using a method that works for you.
Keep a copy of the report and any reply. Follow up if the barrier remains unresolved.

Adapt this example to describe what happened to you. Remove any sections that aren't useful.

What information helps?
Focus on the task, what happened, what you needed and the effect on you. Add a page link, screenshot or technical context only if you can and want to. Report what you experienced without guessing why it happened.
Where your report goes: send it to the organisation responsible for the website or service. No Default does not collect, forward or resolve reports about other organisations.
Sharing is your choice
A first-person account can be valuable without representing everyone who uses similar technology or shares a disability. Do not include sensitive information unless necessary, and consider who will receive your report.
